Stanley Rubenzahl,

Stanley
Rubenzahl founded the Maxon Company in 1955 when, during his tenure
as a Welfare Plan administrator, he identified a need for independent
third party administration services by Union Welfare Plans. Mr. Rubenzahl
was personally responsible for all facets of the business from sales
and marketing to consulting and administration. From its beginning
providing primarily claims administration to local clients, Mr. Rubenzahl
diversified the company’s product offerings and extended its
client base so that now Maxon provides services to plans totaling
in excess of $200 million in assets.
In
1974 Mr. Rubenzahl established one of the first Dental HMO’s
in the United States for a consortium of several St. Louis Unions
and businesses. He provided the physical plant, equipment, and supplies.
He also contracted the professional staff, and provided all administrative
services.
Currently
Mr. Rubenzahl, as CEO, is responsible for long range planning, product
development and pricing. Mr. Rubenzahl is a graduate of Harpur College
(SUNY at Binghamton).

David Rubenzahl,
Esq., President, Maxon Administrators, Inc. & Company Counsel

David
Rubenzahl joined the company in 1979 to manage the Dental Plans in
St. Louis, Missouri. Mr. Rubenzahl was responsible for all facets
of their operation from equipment financing, and supply purchasing,
to provider contracting and plan consulting.
Mr.
Rubenzahl came to New York in 1985 as company counsel and chief consultant.
His expertise is in private and public plan administration, and client
and provider contracting. He also is intimately involved in systems
development insuring compliance with evolving state and federal regulation.
Most recently,
Mr. Rubenzahl developed a NYS GML 207 consulting program to be offered
to governmental Workers' Compensation clients.
Mr.
Rubenzahl, a graduate of Lehigh University in 1974 and the University
of Miami School of Law in 1977, is a member in good standing of the
New York and Florida Bars and serves on the board of the Society
for Professional Benefit Administrators.

Hana Rubin, Chief
Operating Officer and Director of Administration

Ms.
Rubin started at Maxon in 1980 as its Director of Administration.
In that position she was responsible for development of claim and
billing systems for Maxon’s true group and association business.
Along
with her responsibilities as Director of Administration for Maxon,
Ms. Rubin has personal responsibility for Maxon’s administration
of insured statutory disability benefits in the states of New York
and New Jersey. In that capacity she also serves as liaison to the
New York and New Jersey agencies regulating statutory disability
benefits.
Ms.
Rubin graduated from Ithaca College in 1978 with a degree in sociology.
She received her MBA in finance from The University of Connecticut
in 1980.

Ann Romel-Maslak,
Manager of Claims Administration
Ms.
Maslak joined The Maxon Company in 1983 as its manager of claims
administration. In that position, she has overseen the evolution
of Maxon’s claims administration system maintaining the interface
of technology with trained personnel. She instituted Maxon’s
outside audit program and is in charge of training for new administration
business as well as for new hires.
Ms.
Maslak came to Maxon from the Mutual of Omaha Companies where she
began as a claims supervisor in 1978, leaving that company in 1983
after several years as a cost containment specialist.
Ms.
Maslak, fluent in Russian, is a graduate of Western Connecticut State
College in 1975 with a degree in primary education.

Kim Moore, Manager
of Workers’ Compensation Claims
Kim
has been with The Maxon Company since 2005 bringing over 20 years
of experience. As the Manager of Workers’ Compensation Claims,
her responsibilities include handling both medical and lost time
claims. Her role also entails indemnity claim investigation, subrogation
and litigation management.
Kim began
her career at Frontier Insurance Group as a Customer Service Representative
responsible for underwriting Property & Casualty business. She
was later promoted to Medical Only Examiner where she mastered the
investigation and evaluation of Workers’ Compensation cases in
various states.
Shortly
thereafter, she became an Indemnity Examiner which led her to the position
of Indemnity Claims Supervisor. Her primary focus was to manage
and review lost time workers’ compensation adjuster’s cases. Most
recently, she moved into the role of Senior Claims Adjuster.
Kim is a
licensed New York State General Adjuster. Additionally, she is
a member of the Mid Hudson Claims Organization.

Doris Garrett, RN,
Director of Case Management
Doris
joined The Maxon Company in 1996. She began as an Assistant Case
Manager responsible for medical record reviews from hospitals and
healthcare providers. Today, with more than 35 years of nursing experience,
Doris is the Director of Case Management responsible for the overall
management of Maxon’s managed care programs and provider relations.
She offers peer-to-peer consultation providing professional guidance
and is a liaison to patients and doctors with recommendations on
how to best maximize benefits.
Prior
to joining The Maxon Company, Doris was a MDS Coordinator and a Wound
Care Supervisor and infection control nurse at Achieve Rehab & Nursing
Facility. She was also an office nurse at Tri-Valley Medical where
she provided counseling and patient teaching services. She made home
visits to assess compliance with medical regime.
Formerly,
Doris was a nurse for the Sullivan County Department of Public Health.
Here she helped establish a high risk prenatal clinic as well as
working in infectious disease, high risk prenatal and well-baby clinic.
During this time she became a school nurse for Fallsburgh Central
School District where her responsibilities included first aid to
students and faculty, as well as the administration of medication
and procedures for medically compromised students.
Doris,
a graduate of the Middletown Psychiatric Center School of Nursing,
is certified in Community Health Nursing and Coronary Care and Respiratory.
Additionally, she is a member of the New York State Nurses Association
and Emergency Medical Response Team for FEMA.

Ernie Mele, Director
of Marketing and Specialty Programs
Mr.
Mele joined Maxon in 1984 as its Director of Marketing. His expertise
is in the identification and development of insurance company markets,
and specialty programs such as fiduciary liability coverages and
self-funded statutory and traditional short term disability.
Mr.
Mele began his career in the benefits industry as a group accident
and health claims examiner with Northeastern Life Insurance Company
of New York. He progressed from that position through manager of
the claims department to Vice President of the Group Accident and
Health Department until Northeastern discontinued its group insurance
product line in 1976, at which time he joined the CNA Insurance Companies
as Senior Marketing Executive.
Mr.
Mele was appointed manager of the CNA major group sales offices for
New York and New Jersey in 1980.
Mr.
Mele received a BA degree from Hunter College in 1962 and is a past
President of the Group Health Claims Association of New York and
the Group Insurance Association of Greater New York.

Dorothy Barone,
Manager of Information Technologies
Ms.
Barone joined Maxon in 1990 as an application programmer with 10
years of programming experience. She was named data processing manager
in 1995. In this position she manages all aspects of data processing
from data entry and data base management to electronic data interchange.
Ms.
Barone has programmed most of the sorts and edits we employ to create
the standard and personalized reports we provide to our clients,
as well as the check issuance and data base management applications
we currently employ.
Ms
Barone attended SUNY Farmingdale where she received a degree in Business
Administration in 1981.
